New Client to JP Massage Therapy

All clients new to the clinic need to fill out a basic health history and sign a consent form before their first treatment.
Minors (17 and under) require a parent or guardian signature. If you prefer to fill in the paperwork in-person instead, please be sure to show up 10 minutes early to your appointment.

Files are kept in the clinic for FIVE years after your last treatment. If it has been longer than that since you were in, you will need to fill in the intake paperwork again.

Direct Billing

Please be sure to return the completed form to the clinic (in person or via email) before your treatment, preferably at least one day prior. You will only have to do this once, as long as that specific policy is in effect. If the policy is cancelled, the numbers change, or you have a new insurance company, please fill out a new form and resubmit. For more information, go to Direct Billing FAQ
